5th Grade Wax Museum

Hello Lakewood Families,

You are invited to come and visit our Fifth Grade Wax Museum on Thursday, May 14th from 9:00-10:00 am.  

We are limiting 2 adults per student (small children will be allowed) to attend the display.  All visitors must complete and be approved through our Raptor system before May 11th, NO EXCEPTIONS. 

If you have filled out an application with in the last year in the Raptor System, you DO NOT need to fill out a new application. This form must be completed to attend the Wax Museum. 

If you have never filled out an application through the Raptor System, please fill out BOTH.

Please make sure when filling out the application to submit your name as shown on your ID.

Please only fill out the the forms ONE TIME. 

We appreciate your understanding and cooperation in making this event a successful one.

Estás invitado a venir a visitar nuestro Museo de Cera de Quinto Grado el jueves, 14 de mayo de 9:00 a 10:00 am.

Estamos limitando 2 adultos (niños pequeños serán permitidos) por estudiante para asistir a la ceremonia. Todos los visitantes deben completar y ser aprobados a través de nuestro Sistema Raptor antes del 11 de mayo.  NO HAY EXCEPCIONES.

Si usted ha llenado una solicitud en el último año en el sistema Raptor, usted NO necesita llenar una nueva solicitud. Pero por favor complete el siguiente formulario.  Este formulario debe cumplimentarse para asistir al Museo de Cera.

Si nunca ha rellenado una solicitud a través del Sistema Raptor, por favor rellene AMBAS.

Al llenar la solicitud, asegúrese de que su nombre es el que figura en su documento de identidad.

Por favor llene el formulario solamente UNA VEZ.
